Low Income Spay/Neuter Program

 

OVMA recently received funding and is now ready to implement the surgical program. Only OVMA members are eligible for this program. An application must be submitted to the OVMA office and you must receive approval before you can be reimbursed by this program.

PARTICIPATION PROCEDURE:

  • Low income owners apply, show proof of low income, and must receive approval from OVMA before surgeries can be scheduled 
          Approved owner applications include the OVMA logo and will be re-typed for legibility     (See Sample Below)
          Original owner application will be on file at the OVMA office
          Official OVMA seal will be stamped and embossed over the approval signature (If seal is not present, the approval is not valid)
          Approval notices will include a list of participating veterinarians
  • When the owner calls for a surgical appointment, please determine approval date
         Surgery must be performed within 15 days of approval

Do not schedule surgeries unless the owner has an application approved by OVMA
Surgeries performed without an approved application will not be reimbursed

  • Schedule surgery
         Collect and keep $10 co-payment at the time of surgery
  • If it is determined that the animal is unfit for surgery, please issue a medical exemption
         Provide OVMA with a written copy of the medical exemption
  • Provide rabies vaccination for $5 if requested by the owner
  • After surgical completion, sign and date veterinary verification in the confirmation box on the owners’ application
  • Make and keep a copy of the completed owners’ application for your records
  • At the end of the month, complete and submit a monthly invoice (Sample Below), along with copies of completed owner’s application
         Monthly invoice shall include surgical fees that were submitted and approved on the Veterinary Application & Agreement
         Monthly invoice must be notarized
  • OVMA will issue payment for 80% of the submitted and approved amounts
